NEW PLAYERS

The following forms must be completed when registering new players.

• Thistle Commitment Letter
• Thistle Sportsmanship Pledge
• Thistle Medical Release Form
• Thistle Waiver Form
• US Club Registration Form
• NJ Youth Soccer Registration Form - this is not for all new players.  Your coach will inform you if this form is required.

A copy of the players proof of birth (ex: birth certificate or passport) and a 2x2 colored photo will be required.  A digitial photo of your child can also be accepted.  Please speak to you coach for information on how to submit your child’s photo electronically.  Photos are required for player passes.

RETURNING PLAYERS

The following forms must be completed by returning players.

• Thistle Commitment Letter
• Thistle Sportsmanship Pledge
• US Club Registration Form - this form must be completed only if information has changed from the previous season.  For example you have moved or your insurance information has changed.

A 2x2 colored photo will be required. A digitial photo of your child can also be accepted.  Please speak to you coach for information on how to submit your child’s photo electronically.  Photos are required for player passes.

COST

The registration fee for the 2010/2011 season is $225. The registration fee for the 2011 spring season is $175.00.  Additional costs for uniforms and tournaments will apply.
